Community Connect

Case Management

Depending on those unique needs and eligibility, we connect clients to supportive services and ensure that they receive the most effective assistance to improve their lives and secure a brighter future. Our multi-disciplinary team of case managers provide client-centered and trauma-informed care.

Housing Assistance

Through Community Connect, we help individuals and families secure safe, stable housing. Our team offers rental assistance, housing referrals, and ongoing case management to navigate the complexities of housing resources. From homeless prevention to rapid rehousing, Community Connect strengthens the safety net for our most vulnerable neighbors.

Youth Program

Leadership Academy

Investing in our youth is investing in the future. Community Connect’s Leadership Academy is a 17-week program for young adults aged 16-20, offering leadership training, life skills development, and career exploration. The curriculum fosters confidence, vital skills, and community involvement, empowering students to grow personally and become the next generation of compassionate, effective leaders.

Workforce Development empowers clients with tools for economic stability through job training, financial literacy workshops, and educational programs for all ages.
