About Community Connect

Community Connect case managers provide free, high-quality, client-centered support tailored to each household’s unique needs and goals. Through comprehensive assessments and personalized service plans, they guide clients in accessing housing support, food assistance, workforce development, financial empowerment, and other critical resources.

By coordinating care across programs and partners, we remove barriers, streamline services, and empower individuals and families to achieve lasting stability and self-sufficiency.

Who We Serve

Community Connect serves individuals and families facing economic hardship, housing instability, food insecurity, underemployment, and other life challenges. Depending on eligibility and individual needs, clients may be connected to one or more of our supportive programs to ensure they receive the most effective and appropriate assistance.

We are committed to meeting people where they are  with dignity, respect, and without judgment  and walking alongside them as they work toward greater stability and a brighter future.

Homeless Prevention

Our Homelessness Prevention Program protects housing stability by intervening before a crisis escalates. We provide targeted rent and utility assistance, landlord mediation, and connections to essential community resources to help individuals and families remain safely housed.

By addressing the root causes of housing instability  such as income loss, unexpected expenses, and employment barriers  we help clients stabilize their households, preserve tenancy, and regain financial footing. Prevention reduces the trauma and long-term impact of homelessness while strengthening families and our community.

Key Services
  • Financial Assistance: We provide short-term financial support to cover rent and utilities, helping clients stay in their homes during times of crisis.
  • Landlord Mediation: Our team works with clients and landlords to resolve disputes and prevent eviction.
  • Resource Connection: Clients are connected with vital resources such as job training, healthcare, and financial literacy programs to address the underlying issues contributing to their housing instability.
  • Personalized Case Management: Each client receives individualized support from our case managers, who develop tailored action plans to help them achieve long-term housing stability and self-sufficiency.

  • Low-income individuals and families facing imminent risk of eviction or homelessness.
  • Clients must meet specific income and housing criteria to qualify for assistance.

Housing Opportunities for Persons with AIDS (HOPWA)

Description

The Housing Opportunities for Persons with AIDS (HOPWA) program provides critical housing assistance and supportive services to individuals and families living with HIV/AIDS. Stable housing is essential to maintaining health, dignity, and long-term well-being.

Funded by the City of Las Vegas, HOPWA allows us to prevent homelessness among those at heightened risk due to medical needs, stigma, and financial instability. Through rental assistance, housing stabilization, and case management, we help clients remain safely housed while improving overall health and stability.

  • Supportive Case Management
  • Short-Term Rent, Mortgage, and Utility Assistance (STRMU)
  • Tenant-Based Rental Assistance (TBRA)
  • Permanent Housing Placement (PHP)
  • Other Supportive Services including, but not limited to, all TJOP wraparound services, nutritional education, affordable groceries, transportation, assistance in gaining access to mainstream benefits and workforce services.
Client Eligibility
  • Must reside in Clark County
  • Medically diagnosed with HIV/AIDS
  • Low-income persons (at or below 80 percent of area median income)

Shared Housing

Description

Our Shared Housing Program provides a strategic and supportive housing solution for individuals seeking safe, affordable living arrangements. By thoughtfully matching residents based on compatibility, lifestyle, and individual needs, we create stable, community-centered environments that promote accountability, reduce isolation, and strengthen overall well-being.

We collaborate with trusted landlords and community partners to secure quality, affordable housing options that meet safety and stability standards. Beyond housing placement, we provide ongoing case management and coordinated access to essential services  including workforce support, financial empowerment, and other stabilization resources  to help residents maintain housing and progress toward long-term independence.

Shared Housing is more than a place to live; it is a pathway to stability, connection, and sustainable self-sufficiency.

Rapid Rehousing

Description

The Rapid Rehousing Program swiftly transitions individuals and families from homelessness into safe, permanent housing. Through short-term rental assistance and intensive case management, we not only secure housing but help clients break down barriers to lasting stability. We address critical challenges such as workforce development, financial literacy, and income enhancement, empowering clients with the tools they need to maintain housing and rebuild their lives. By partnering closely with landlords and property managers, we secure affordable options and advocate to remove housing barriers. This approach not only ends homelessness but fosters long-term stability and independence.
