About Case Management

The Just One Project’s Case Management Services provide individualized support to help clients navigate challenges and access the resources they need to build stability and self-sufficiency. Our case managers work closely with clients to assess needs, develop personalized service plans, and connect them to food assistance, housing support, workforce services, and other essential resources. Through ongoing guidance, advocacy, and coordination with community partners, we aim to remove barriers and support clients in achieving their goals.

  • Intake and comprehensive needs assessments
  • Individualized service and goal planning
  • Food assistance and basic needs resources
  • Housing navigation and stability support
  • Workforce and employment referrals
  • Connection to benefits and public assistance programs
  • Coordination with internal programs and community partners
  • Ongoing follow-up, advocacy, and progress monitoring

TJOP case managers work one-on-one with clients to provide guidance, advocacy, and connections to essential resources—supporting stability, dignity, and long-term self-sufficiency.
