Community Connect case managers are dedicated to understanding each client’s unique needs and providing personalized support. By conducting thorough assessments, they identify the resources and services that can best help clients achieve stability and success.

Depending on those unique needs and eligibility, clients may be linked to one of many supportive services we offer, to ensure that they receive the most effective assistance to improve their lives and secure a brighter future.

Homeless Prevention

Our Homeless Prevention program is dedicated to keeping individuals and families in their homes by offering financial assistance for rent and utilities, mediation with landlords, and access to essential resources.

We address the root causes of housing instability to prevent homelessness before it starts, ensuring that our clients can maintain secure and stable living conditions.

Key Services
  • Financial Assistance: We provide short-term financial support to cover rent and utilities, helping clients stay in their homes during times of crisis.
  • Landlord Mediation: Our team works with clients and landlords to resolve disputes and prevent eviction.
  • Resource Connection: Clients are connected with vital resources such as job training, healthcare, and financial literacy programs to address the underlying issues contributing to their housing instability.
  • Personalized Case Management: Each client receives individualized support from our case managers, who develop tailored action plans to help them achieve long-term housing stability and self-sufficiency.

  • Low-income individuals and families facing imminent risk of eviction or homelessness.
  • Clients must meet specific income and housing criteria to qualify for assistance.

Housing Opportunities for Persons with AIDS (HOPWA)

Description

HOPWA provides vital housing assistance and supportive services to individuals and families living with HIV/AIDS.

Funded by City of Las Vegas, HOPWA allows us to assist clients by preventing homelessness. People living with HIV/AIDS are often at heightened risk of homelessness due to health complications, stigma, and the resulting financial instability.

  • Supportive Case Management
  • Short-Term Rent, Mortgage, and Utility Assistance (STRMU)
  • Tenant-Based Rental Assistance (TBRA)
  • Permanent Housing Placement (PHP)
  • Other Supportive Services including, but not limited to, all TJOP wraparound services, nutritional education, affordable groceries, transportation, assistance in gaining access to mainstream benefits and workforce services.
Client Eligibility
  • Must reside in Clark County
  • Medically diagnosed with HIV/AIDS
  • Low-income persons (at or below 80 percent of area median income)

Shared Housing

Description

Our Shared Housing program offers a supportive, communal living arrangement for individuals seeking affordable housing. By matching clients with compatible housemates, we create a safe, stable, and cost-effective living environment that fosters community, reduces isolation, and provides access to shared resources.

Our strategy focuses on creating a supportive and sustainable living environment by carefully matching residents based on compatibility and needs. We partner with local organizations and landlords to secure safe and affordable housing options.

Alongside housing, we provide case management and access to essential services that support residents in maintaining stable housing and working towards their personal and financial goals.

This approach ensures that residents not only have a roof over their heads but also the support they need to thrive in their living situation.

Rapid Rehousing

Description

The Rapid Rehousing Program are designed to quickly transition individuals and families from homelessness to permanent housing. By providing short-term rental assistance, case management, and supportive services, the program helps clients secure stable housing, regain stability, and rebuild their lives.

This program focuses on rapidly connecting individuals and families with housing while offering the necessary support to maintain it. We work closely with landlords and property managers to identify available and affordable housing options and advocate to eliminate barriers to housing that clients face.
