About Case Management
The Just One Project’s Case Management Services provide comprehensive, client-centered support designed to help individuals and families navigate complex challenges and achieve long-term stability. Our case managers conduct thorough needs assessments to identify immediate concerns as well as underlying barriers, and work collaboratively with clients to develop individualized service plans tailored to their unique circumstances and goals.
Through coordinated access to food assistance, housing stabilization, workforce development, financial empowerment resources, and other critical community supports, we help clients address urgent needs while building pathways toward self-sufficiency. Our team provides ongoing guidance, advocacy, and accountability to ensure clients remain connected to appropriate services and make measurable progress over time.
By fostering strong partnerships with community organizations, government agencies, and local service providers, we streamline referrals, reduce service gaps, and promote a holistic approach to care. Our goal is not only to meet immediate needs, but to empower clients with the tools, resources, and support necessary to overcome barriers, strengthen resilience, and achieve sustainable outcomes.

Youth Case Management Services
The Just One Project’s Youth Services program provides focused, one-on-one support for young people ages 15–24, equipping them with the tools, confidence, and resources needed to navigate critical life transitions and build a strong foundation for adulthood.
